Two days: the city, and nothing else
Enough for Medellín, not for Antioquia. Day one the historic centre and the Pueblito Paisa at sunset; day two Comuna 13 in the morning and a free afternoon in Provenza or Laureles. With two days Guatapé does not fit: it is ten hours and would cost you the city you are sleeping in.
Three days: the sweet spot
This is what we recommend to most travellers. City, Comuna 13 and a full day in Guatapé. Each plan gets its own day and none of them is rushed. If recent history interests you, the Pablo Escobar tour fits into the afternoon of day two and gives context to everything else.
Four or five days: Antioquia opens up
One extra day brings paragliding at San Félix in the morning, or a town like Santa Fe de Antioquia or Jardín. Five days brings Hacienda Nápoles, which is a fourteen-hour day and will not share the schedule with anything.
A week: slowing down
From day five the trip changes character: it stops being about seeing more and starts being about living in the city. A market, a football match, a cooking class, a day with no plan in Laureles. Medellín is far better when you stop treating it as a checklist.

For the city plus one day trip, comfortably yes. What three days does not cover is the rest of Antioquia.
Is a full week worth it?
If you have come a long way, yes. The city has its own rhythm and a week lets you meet it without ticking boxes.
What is the best day for Guatapé?
A weekday. On Sundays and public holidays both the road and the town fill up, and the drive back takes noticeably longer.
Can I do everything in two days?
Not without rushing. With two days you have to choose: the city properly, or half the city plus Guatapé.
