Two days are enough to get to know Medellín, not the whole of Antioquia. The key is deciding before you land whether you want the full city or half the city plus Guatapé.

Day 1. The centre in the morning —Botero Plaza, the Metropolitan Cathedral, the Palace of Culture— and Pueblito Paisa late in the afternoon, as the valley lights up. That is exactly the route of the Medellín City Tour, 4 to 5 hours with hotel pickup.
Day 2. Comuna 13 early, when the murals look best and the escalators are quieter (3 to 4-hour tour). The afternoon is free for Provenza or Laureles, or for the Pablo Escobar History Tour, which puts the morning into context.
Booked separately, the City Tour and Comuna 13 add up to USD $220 per vehicle. If you want both on the same day with one car, we quote a closed price.
Day 1. Comuna 13 in the morning and the centre at midday. You see the essentials in one long day, as we describe in Medellín in one day.
Day 2. Guatapé, with nothing else on top: leave between 7 and 8 a.m., climb the Piedra del Peñol early, lunch in the village and head back late in the afternoon (USD $200 per vehicle, up to 4 people).
This option drops Pueblito Paisa and the slower pace, but it is the one people choose when they are not planning to come back to Antioquia soon.
Hacienda Nápoles (13 to 14 hours), paragliding at San Félix and the coffee farm each need their own half or full day. If one of them matters to you, look at the 4-day plan or the 3-day plan.
José María Córdova airport is in Rionegro, 45 to 60 minutes away. That drive eats into your two days, so plan around it: the private transfer is USD $55 per vehicle, up to 4 people, with flight tracking.
