In Medellín it almost never rains all day: it rains in spells, mostly in the afternoon, and mornings are usually clear. With that rule, a rainy day is easy to plan without losing the trip.

The rainiest months are April, May, October and November; October is usually the wettest. Even then, rain comes in spells, mostly in the afternoon, and rarely lasts all day. So the rule is simple: outdoors in the morning, indoors in the afternoon. We go month by month in the best time to visit Medellín.
Comuna 13 is better early anyway, with better light and fewer people. Guatapé also works in the rainy season if you leave between 7 and 8: you climb the rock before the sky changes and walk the village afterwards (USD $200 per vehicle, up to 4 people).
Our tours are in a private car and the driver waits at every stop, so a downpour is spent inside the vehicle and then you carry on. The Medellín Shopping Tour is almost entirely indoors, and the Medellín City Tour (USD $110 per vehicle, up to 4 people) can be reordered to save the interiors —the Cathedral, the Palacio Nacional— for the rainy hour.
The exception is paragliding: flights depend on wind and weather. If flying is not possible that day, we reschedule at no cost or turn it into a viewpoints-and-food outing, whichever you prefer.
A light rain jacket or a small umbrella, closed shoes and a long sleeve for the evening. And sunscreen even when it is cloudy: at 1,500 metres the sun burns more than it seems.
